Sacopee Valley Health Center is a Federally Qualified Health Center located in Porter serving Oxford, Cumberland, and York Counties, in Maine, and Carroll County in New Hampshire. We offer integrated, patient centered Primary Care, Dental, Podiatry and Behavioral Health Services, including a Medication Assisted Treatment (MAT) Program. Sacopee Valley Health Center offers outreach and enrollment services and provides van transportation for its patients.
